Details of the record keeping system:

Medical records are permanent form of information preserved about a patient or client. It is the account compiled by physicians and other health care professionals of a variety of patient health information such as patient's assessment findings, treatment details etc. These information are preserved to make accessible to users for health planning, research and education. Good record and information management system are essential to effective and better clinical and hospital management.

How the records could be retrieved:

In medical records department, we preserved patient's record in both system

Paper based record

Computer based record:

After discharged of Inpatient , first of we enter the basic variable in computer, like as MRN(medical record number),DOA, DOD, Thana, District, Diagnosis, Discharged Mode, Mode of delivery, Babies weight, Babies sex.
and then we preserved patient's discharge file according to medical record number on the shelf.

Each and every patient of ICMH has unique MRN both in IPD and OPD.
When we retrieved discharged file, we use MRN for searching file.

In outdoors, we can access previous information of a patient from computer by using MRN.
